Thinking about the end of life isn’t about focusing on death—it’s about legacy and honoring the life you’ve lived.

Deepening the Journey – Use these prompts as a worksheet to explore the personal values and legacy you wish to leave behind. There is no need to rush; answer only what feels right today.

• Defining Comfort: When I imagine a day that feels peaceful and “like me,” what does it look like? (What are you wearing? Is there music playing? Can you smell cookies baking or the gardenias in the garden, and who is sitting by your side?)
• A Message of Love: If I could summarize my “soul-traits” or the lessons I’ve learned in one sentence for my loved ones, what would it be?
• The Power of Connection: Is there a specific person with whom you wish to share a final word of reconciliation or deep gratitude?
• Honoring My Story: Beyond the milestones, what is one “small” memory—a favorite recipe, a hobby, or a habit—that I want my family to preserve if possible?
• My Advocacy: What is the most important thing I want my Healthcare Proxy to know about my definition of “quality of life”?
